Needed Information to Request Service
Have all this information collected before completing a sensor request(link is external).
Requestor and Billing information:
- Requestor name
- The building name, floor, and room where the freezer/cold room is located
- The COA that will be used for the cost of the sensor and the monthly monitoring support cost
- Billing contact
Freezer/Cold Storage Information:
- Cold storage ID (EHS number)
- Freezer/cold room type: -80, -30, -20, LN/Dewar, cold room, other
- Freezer/cold room serial number
- The model number of the freezer/cold room
- Freezer/cold room contact information
- Principal investigator/responsible party
- Phone
- Address
Sensor and Alert Settings:
- People who will receive alerts
- Contact Name
- Preferred Contact Methods (E-mail, Text, HTML)
- Mobile Number
- Phone Carrier (so we can configure the message on that carrier)
- When should this person be contacted (primary contact, secondary contact, tertiary contact). Standard time lag between the level of contacts is 15 minutes
- Sensor Settings
- Maximum and minimum temperature change from the normal temperature (Celsius)
Sensor Pick Up or Delivery:
please note you can either pickup the requested sensor(s), or request University’s TR&S Courier Service to be shipped to you
- Pickup sensor(s) at the TSSC - Technology Support Service Center (Sterling Hall of Medicine Medical Library 333 Cedar St, New Haven CT, Room LE-20)
- Ship to you: Request University TR&S Courier(link is external) to have the sensor(s) being picked up at TSSC and delivered to you after you receive a confirmation email to inform you that the sensor(s) are ready. Shipping fees apply and may incur a delay in sensor(s) receipt.
Current Status on Rollout of Service:
We are currently in a soft phase of rollouts targeting the departments/labs that has more priority in needs. We will communicate with all existing freezer owners in near future.
