Needed Information to Request Service

Have all this information collected before completing a sensor request.

Requestor information:

• Requestor name
• The building name, floor, and room where the freezer/cold room is located
• The COA that will be used for the cost of the sensor and the monthly monitoring support cost

Freezer/Cold Storage Information:

• Cold storage ID (EHS number)
• Freezer/cold room type: -80,-30,-20, LN/Dewar, cold room, other
• Freezer/cold room serial number
• The model number of the freezer/cold room
• Freezer/cold room contact information
• Principal investigator/responsible party
  • Phone
  • Email
  • Address

Sensor and Alert Settings:

• People who will receive alerts
  • Contact Name
  • Preferred Contact Methods (E-mail, Text, HTML)
  • Mobile Number
  • Phone Carrier (so we can configure the message on that carrier)
  • When should this person be contacted (primary contact, secondary contact, tertiary contact)
• Sensor Settings
  • Acceptable time lag between primary, secondary, and tertiary alerts are sent to contacts.
  • Average temperature of the sensor (Celsius)
  • Maximum and minimum temperature change from the normal temperature (Celsius)
  • Door open or Ajar time delay before an alert is sent (minutes)

Current Status on Rollout of Service: 
We are currently in the final testing phase with limited rollout. When service is available, we will communicate with existing freezer owners.
